- contact information of your last dental office to facilitate the transfer of recent radiographs (X-rays)
- insurance information
- whether you would like to visit the dentist for one issue, or for a comprehensive examination
WHAT TO EXPECT AT THE FIRST APPOINTMENT
We are conveniently located on the ground floor of Panorama Place (in the same building as Blue Pearl Mortgage) at 152 Street near Highway 10. There are marked visitor parking stalls in the plaza parking lot.
If you have not already, you will be asked to complete the patient registration, medical and dental history forms. Additional safety processes related to the COVID-19 pandemic can be found here.
You can expect the dentist to review your medical and dental histories, collect diagnostic records, examine your mouth and discuss their findings and recommendations with you.
