MAKING AN APPOINTMENT
To book a new patient exam, submit the completed contact form at the bottom of this page, book online, or speak to a staff member at (604) 372-2882. Please indicate whether you would like a comprehensive exam, or an exam for a specific issue.
The following information will be helpful to our team at the time of booking:
- contact information of your previous dental office to facilitate the transfer of recent X-ray images (radiographs)
- insurance information
OFFICE LOCATION
Vivant Dental is located on the ground floor of Panorama Place located near the southeast corner of the 152 Street and Highway 10 intersection in Surrey. (We are in the same building as Blue Pearl Mortgage.)
There is free parking in the plaza. Please observe the parking signs.
WHAT TO EXPECT AT YOUR FIRST VISIT
You will be asked to complete patient registration, medical and dental history forms on arrival. If you have a list of medications, please bring that along.
During your new patient examination, your dentist will review your medical and dental histories, collect diagnostic records, examine your mouth and discuss their findings and recommendations with you. For insurance and payment information, click here.
